  New IRB Paper Forms
  

  The IRB is now using InfoEd, and online system, for submitting retrospective research protocols. However, all other protocols still use the paper forms from the IRB's website. Some of those forms have been updated. The presentations here will walk you through the changes on the IRB's paper forms.

  For InfoEd Users

  You must register to access the InfoEd reference materials on this site.

  This site contains proprietary information about InfoEd. We are obligated to protect that information. The first time to try to access an InfoEd folder on this site, you will be asked to create a username and password. Registering for this site is as easy as registering for any other Web site.
  

  Reference Materials

  When you access one of these folders for the first time, you will be asked to "subscribe" to the folder, so that you will be notified when materials are added or updated.

  Folder	Description
  Essentials	This page contains information that is useful for almost all InfoEd users. Especially, how to create and format pdf files for uploading to InfoEd, and how to give someone else access to your work in InfoEd (that is, creating a delegate).
  Human Subjects Module	This page contains written instructions and videos that show you how to create a human subjects protocol in InfoEd.
  Lab Animals Module	This page contains written instructions and videos that show you how to create a lab animal protocol in InfoEd.
  Proposal Development Module	This page contains written instructions and videos that show you how to create a grant proposal in InfoEd.
  InfoEd Forms	This page contains forms that you submit online: New User, New Personnel, and New Subcontractor.
